Medical Form: Needed yearly for your child's file at preschool. Take this blank form with you to their medical exam and the physician will complete.

ODJFS Form: This is to be on file at preschool and will be collected at the beginning of the school year.

Application/Registration Form: This is completed and returned with registration fee to secure a spot in our preschool. We take applications for current students, past students, siblings of past students and church member's children beginning January 3, 2007. Open enrollment begins March 1, 2007.
